Access and continuity are at the core of the Patient Centered Medical Home model. The goal is for patients to have timely access to care and information when needed from their chosen primary care practice. This includes expanded access through alternative methods like telephone or email consultations in addition to in-person visits. It also means patients receive coordinated care from a personal physician they know and trust over time. Continuity fosters strong physician-patient relationships built on trust and understanding of the patient's needs and preferences.
